Get Ready for Back to School

Supply lists are at the bottom of this page.

All returning students need to fill out the following forms.

This form requesting or declining transportation must be filled out each school year for every student.

2. Educational Benefits Form

Formerly the Free & Reduced Lunch Form, this form is encrypted for your security.
Only ONE (1) form is needed per household. Paper copies are available at your school front office.

Important change for School of Choice students:
Returning School of Choice students are no longer required to fill out School of Choice paperwork every year. Accepted applications are now valid for multiple years.
